You have the right to obtain your medical records, including doctors’ notes, medical results and other documentation related to your care.
You have the right to participate in decisions about your care and treatment and to give informed consent before any procedure or intervention.
You have the right to refuse any treatment or procedure that you do not wish to receive and to be informed of the possible consequences of your decision.
You have the right to privacy and confidentiality of your personal and medical information and to access, amend or restrict the use of your information as permitted by law.

You have a responsibility to treat our practitioners and providers with courtesy and respect and to follow the rules and policies of the facility.
You have a responsibility to provide accurate and complete information about your health history, current condition, insurance and any other matters related to your care.
You have a responsibility to ask questions if you do not understand your diagnosis, treatment plan, medications or discharge instructions and to follow the agreed-upon plan of care.
You have a responsibility to accept the consequences of your decision and to respect the rights of other patients who may wish to receive the same treatment or procedure.
You have a responsibility to respect the privacy and confidentiality of other patients and staff and to notify us if you believe your information has been accessed or used inappropriately.
You have a responsibility to cooperate with the process of investigating and resolving your concerns, complaints or grievances and to provide feedback or suggestions for improvement.
